33, St James Square, Holland Park, London. Private 1519, Surrey Yeomanry Jack Yates, a native of Prescot (address unknown) attested for 4 years Territorial Service on 17th March 1911 in Croydon. He was 21 years old and worked as a Clerk for United Carbide. His next of kin was his wife, Mrs J Yates, of Hollywood, Henley-on-Thames.
He attended annual camp in 1911 through to 1914 and was then embodied to the 3/1st Battalion on the outbreak of war in August 1914. He was transferred to the 2/1st Battalion on 26th June 1915 and then moved to the 45th Provisional Battalion on 9th July the same year.
He was discharged, having served for 5 years, on 16th March 1916. He did not serve overseas and therefore had no medal entitlement.
